Increased bile duct complications and/or chronic rejection in crossmatch positive human liver allografts.

چکیده
B ILE duct complications continue to be a significant cause of bile morbidity after orthotopic liver transplantation, with an incidence of 7% to 34% of patients. 1•2 Bile of duct obstruction, usually due to strictures, biliary leakage, and/or cholangitis, accounts for the majority of these complications. In contrast. although low incidence of chronic rejection has been reported under FK 506-based immunosuppression in early trials of liver transplantation,) chronic rejection after liver transplantation occurs in only 5% to 9% of cases. and it remains one of the most common causes of graft loss.~·5 There have been many conflicting recent reports concerning a relationship between the bile duct-related complications or ductopenic chronic rejection and a positive crossmatch.6 s. We investigated the outcome of liver grafts from positive crossmatch donors. focusing on biliary complications and ductopenic chronic rejection, and we compared the results to a consecutive negative crossmatch control group at the same time.

A histometric analysis of chronically rejected human liver allografts: insights into the mechanisms of bile duct loss: direct immunologic and ischemic factors.
Conspicuous pathologic features of chronic liver allograft rejection include bile duct loss and chronic obliterative arteriopathy. A quantitative histometric analysis was performed to document the extent of bile duct loss, the size of the "vanished" ducts and the extent of chronic obliterative arteriopathy and to determine whether there was any relationship between chronic obliterative arteriop...
متن کامل- - _ . - - - - - - ~ 75 The antibody cross match in liver transplantation
Six hundred sixty-seven first, second, and third orthotopic liver allografts in 520 patients were reviewed to determine the effect of recipient panel-reactive antibody (PRA) and donor-recipient antibody crossmatch on 2-year patient and liver allograft survival rates. Neither a high panel-reactive antibody nor a positive crossmatch for donor-specific preformed antIbody was associated with decrea...
متن کاملChronic liver allograft rejection and obliterative arteriopathy: possible pathogenic mechanisms.
CHRONIC REJECTION is a major cause of late liver allograft failure. Generally accepted pathologic features of this condition include chronic obliterative arteriopathy (COA) and bile duct loss. The mechanisms involved in graft damage are poorly understood. Bile duct loss has been attributed to direct Iymphocytotoxic attack, which seems reasonable considering the histologic appearance of rejectio...
متن کاملHumoral rejection associated with antidonor lymphocytotoxic antibodies following liver transplantation.
H EPATIC allografts are more resistant than other solid organs to antibody-mediated rejection, and human hepatic transplants can be successfully performed in the face of donor-specific lymphocytotoxic antibodies (positive crossmatch).1.2 However, we recently reported a significant decrease in patient and graft survival when a liver transplant was performed against a positive IgG lymphocytotoxic...
متن کاملAssociation between donor-recipient HLA-DR compatibility and cytomegalovirus hepatitis and chronic rejection in liver transplantation.
P REVIOUS studies in liver transplant patients have demonstrated that liver allografts matched for HLA. especially HLA-DR, have lower survival rates, raising the concept that HLA plays a dualistic role in liver transplan-tation. HLA matching will reduce rejection but may augment other major histocompatibility complex (MHC) restricted cellular immune mechanisms of liver allograft injury. 1.2 The...
